Gout
In the past, gout was the disease of kings or the rich. Because only those who had enough money could afford a sumptuous diet with meat and alcohol. Today, gout is one of the most common diseases of affluence, along with diabetes. In Germany alone, one to two percent of adults suffer from it.
Gout is a metabolic disease in which too much uric acid accumulates in the blood, which is produced when so-called purines are broken down. These in turn are a building block for many important substances in the body, for example as parts of the cell nucleus. Above a certain concentration, the uric acid can no longer dissolve in the blood – instead it is deposited in the form of crystals in various parts of the body, especially in joints and tendons, but also in bursae and internal organs such as the kidneys.
Causes gout
Gout is triggered by too much uric acid in the blood. There are two known causes for this: Either the body forms too much uric acid or it breaks down too little. In both cases, the balance of the metabolism is disturbed. The excess uric acid crystallises and is then deposited in the body. This leads to painful inflammations that are accompanied by permanent changes in joints or organs.
The most common form is hereditary primary gout, in which the body excretes too little uric acid. The rarer secondary gout is caused by another underlying disease (kidney damage, tumour, untreated diabetes). But certain medicines can also trigger gout as a side effect.
Symptoms
An increased uric acid level in the blood is not noticed at first. Only when a critical concentration is exceeded and the joint becomes inflamed as a result, does the disease become visible and noticeable. Initially, in an acute attack of gout, the metatarsophalangeal joint of the big toe swells, becomes red, hot and painful to the touch. Other base joints of the knee, foot or thumb can also be affected. If gout is not treated, over time it becomes chronic gout, with permanent joint inflammation, pain at rest or during movement, joint deformities and permanent damage to organs.
Diagnosis
Doctors often recognise acute gout by the painful, reddened swelling at the base or end joint of the big toe. In order to clearly detect gout and rule out other possible causes (e.g. an infection, arthritis or arthrosis), a joint puncture is necessary. Fluid is taken from the joint with a fine hollow needle. If there are uric acid crystals in the fluid, it is clearly gout.
Determining the uric acid level in the blood alone is not conclusive enough, as it often drops into the normal range during a gout attack.
Treatment gout
It is important to lower the elevated uric acid level in the blood. A healthy low-purine diet is indispensable here. Purines are mainly found in meat, offal and sausage, in fish and shellfish, but also in pulses, spinach and asparagus. In addition, alcohol – especially beer and spirits – should be avoided. Daily drinks should consist of about two litres of water or unsweetened tea. All these measures help the metabolism to break down uric acid and excrete it more effectively.
